Azi,
The story goes on the Apta Rav as Grand Rebbe pointed out who was originally buried in Eastern Europe (not sure the city, perhaps Mezibuzh is right, but I taought Iasi).

The diffrence is that the tale there is about Malachim (angels) who moved him and woke up the Shomer (Cemetary Guard) in middle of the night in Tiverya so they will be able to complete to move.

This story is a long ongoing debate on the truth of it, I think it was the Munkatcher Rav (Rabbi Eluzer Schapiro Baal Minchas Eluzer) who commented "if the Malachim were able to do the entire move on there own, just for the openening of the Cemetary they had to wake up the Shomer?".....

That is the reason of the dual Kevarim.

There are some people who say that a rebbe buried in Mezhibuz (can't remember which one now) was moved to Tiveriah's cemetery. There are actualy two graves for him. I saw the one in Tiveriah. People daven at both kevarim.

I once heard from Rabbi Oelbaum (KGH) that once a person is moved, although the grave is not reused the practice is to no longer daven at the old kever.

The rebbe was originally buried in Cleveland OH where he passed away while traveling to Chasisdim in that city, and was later moved by his family to what they considered a more respectfull rest-place.

בנימין,
לבד היותו חתן הדברי שמואל היה לו יחוס עצמו בהיותו נכד בא"ב לאדמור"י לעכאביץ ולמעב"ק
