” ish horowic ” but ” ish halevi horowic” never heard of

Horowitz is the name of a city, that’s where the title “Ish Horowitz” stems from, meaning a man from the city Horowitz!

usually the einiklach from the shloh hakodoish are called ish horovic , and as the are levijim its normal :
avrohom ben moshe halevi, ish horovic
but here it is like : moshe ben avrohom ish, halevi horovic
