His death certificate can be found at the NYC vital records archives in Tribeca. I’ve seen it listed, next time I go I will get a copy of it. Should provide more info re who his father was.

Most probably a descendant of one of Reb Chaim Volozhiner’s grandsons, and a cousin of the Bais Halevi.
