Interestingly his name is Levin although he was a cohen.
Even more interesting, He was Rov in Uzda. R’ Moishe Feinstein (or hi father, or both?) was also Rov there!

It says on the tombstone that Rabbi Levin was the rabbi of the Beis Midrash HaGadol. This was also known as the Crawford Street Shul.

It seems that the shul is no longer functioning.
My grandfather Rabbi Moshe Silman of MN was also born in Uzda. My grandfather has smicha from Rabbi Levin, which was later signed by Reb Moshe Feinstein.

Rabbi David Feinstein (Rabbi Moshe’s father) was the rabbi of Uzda until 1917,
When Rabbi David Feinstein moved to the Strobin rabbinate, his place was filled by two rabbis – his brother-in-law Rabbi Yaakov Kantrowitz, and Rabbi Yitzhak Yaakov Levin.
In 1917, Rabbi Moshe Rabbi Moshe Feinstein was appointed rabbi of Uzda.
