I remember him from the Lower East Side.

That’s a very broad question, I was a little kid in 60?s and attended RJJ my father once took me to the Rebbe for a blessing.
The text of the matzeiva does not support the fact that he was the SLovakia Rebbe

Are you saying that this in not the kever of the Slovakia Rebbe?
It shoud read Sokolivke and not Slovakia.
I think the text on the Matzeivah does pretty much support the fact that he was the Sokolivke Rebbe. On a side note. The Matzeivah does say that he was a direct descendant from Rav Meir of Premishlan ZT”L

The Rebbe’s shul on 156/8 Rivington Street
Rabbi Halpern lived on the first floor of 156 Rivington Street, New York, NY 10002. He held shabbas services in the living room of his apartment. His daughter married Rabbi Singer, who became the Rabbi of the Willet Street School.
I lived in Rabbi Halpern’s building on the third floor. He held the services from at least 1944 to 1957, when I got married.
